Our Life Transitions program supports young people aged 16–25 to explore their options, identify their goals, and build the skills and confidence needed to take their next steps, whatever those may look like.

 

Whether you're finishing school, considering further study, looking for work, developing independent living skills, or still figuring out what comes next, we can work alongside you to create a plan that reflects your interests, strengths, and aspirations.

Some young people may be ready for university, some may be looking for work, some may want to start a business, and others may simply need time and support to explore their options. Our role is not to tell you what your future should look like. Our role is to help you discover what matters to you, identify opportunities, build practical skills, and develop the confidence to move towards the life you want. We work alongside young people and, where appropriate, their families, schools, therapists, employers, and other supports to create a coordinated approach that helps turn goals into achievable next steps. Because there is no one right path after school—only the path that is right for you.
